REPORTINGOFDEFECTS (CAR 248)

At the terminationof eachflight, or in anyurgent case, during the currencyof

theflight, youmust report all defects in the aircraft, aerodromes, air routes, air

route facilitiesor airway facilitieswhichhave come to your notice.

Where a defect in the aircraft is reported in accordancewith the above

paragraph, the operator of the aircraft shall take such action in relation thereto

as is requiredunder theRegulations.

INTRODUCTION

TheAustralian Transport SafetyBureau (ATSB) isAustralia’s national safety

investigator for aviation incidents, accidents and safety deficiencies. As an

independent CommonwealthGovernment statutory agency, theATSB is

governed by aCommission and is entirely separate from transport regulators,

policymakers and service providers.

TheATSB’s function is to improve safety and public confidence in the

aviation,marine and rail modes of transport throughexcellence in:

• independent investigation of transport accidents and other safety

occurrences

• safety data recording, analysis and research

• fostering safety awareness, knowledge and action.

TheATSB is established by the Transport Safety InvestigationAct 2003 (TSI

Act) and conducts its investigations in accordancewith theprovisions of the

Act. Under the TSI Act, it is not a function of theATSB to apportion blameor

provide ameans for determining liability. TheATSBdoes not investigate for

the purpose of taking administrative, regulatory or criminal action.

Thepurposeof theATSB’s investigations is to enhance aviation safety

through the identificationof safety issues.

TheATSB’s accident investigation role is a fundamental part of Australia’s

transport safety framework. Lessons arising from the investigations
